Children’s Community Nursing Team

The Service:
The CCNT provides a paediatric specialist nursing care to children and young people in their own homes and in community settings such as nurseries and schools across Lambeth, Southwark and Lewisham. This service aims to facilitate earlier discharge from hospital, prevent unnecessary readmissions and reduce the number of visits a family are required to make. We liaise regularly with all members of the multi-agency team, based both in hospital and community settings in order to ensure the best service for children and families in the community and the safest and most effective discharge home.
Who is the service for?
The CCNT will see children aged between 0-16yrs and children and young people with disability up to the age of 19yrs who are still in full time education. All children and young people referred to the service must be registered with a GP in Lambeth, Southwark or Lewisham
Criteria for access to service
The service can deliver nursing care for the following health needs:
Drug administration including IV antibiotics and some chemotherapy, wound dressings, blood tests, monitoring treatment progress e.g. blood pressure monitoring, tracheostomy care, nasogastric or enteral feeding devices, oxygen therapy and saturation monitoring, sleep studies, teaching and education of nursing needs described above, end of life care
Accessibility of this service
This service operates 365 days a year including public holidays between the hours of 9am and 5pm

Service provided in the community by appointment with CCNT, 24hrs notice of a visit required

How is this service funded?
This service is facilitated by Lewisham and Greenwich acute trust and commissioned by Southwark CCG via a service level agreement.
